On September 13, China's National Healthcare Security Administration (NHSA) published a notice on its official website outlining a typical case of commercial bribery in pharmaceutical and medical-device procurement. The notice revealed that staff at the Second Hospital of Hebei Medical University had taken bribes on 46 occasions over an 11-year period, totaling about 184 million yuan (roughly $25.9 million). The case exposes serious weaknesses in internal controls at some medical institutions, as well as deeply entrenched kickback networks in pharmaceutical and medical-device procurement.
The NHSA said the case came to light through an analysis of court judgments involving commercial bribery in pharmaceutical and medical-device procurement over the past three years. Initial evidence emerged from a separate criminal case involving a Shanghai-based trading company that supplied cardiac stents and balloon catheters. That case exposed a scheme in which staff at the Hebei hospital received kickbacks of 5,000 yuan for each stent. In connection with its sales of medical consumables to the cardiovascular department at the hospital's East Campus, the company and its controlling shareholder paid a total of 14.0333 million yuan in cash kickbacks to the then-deputy head of the hospital, the head of its cardiology department and others. The arrangement called for a 5,000-yuan kickback for each stent and 1,000 yuan for each balloon catheter costing more than 4,000 yuan. After that case came to light, nine additional criminal judgments involving individuals connected to the hospital took effect in the second half of 2025 alone. Piecing these cases together revealed the full extent of the misconduct: hospital staff had taken bribes on 46 occasions, with the largest single payment reaching 16.58 million yuan and the longest-running instance of bribery spanning 11 years. The bribes totaled 184 million yuan.
The notice states that more than 30 pharmaceutical and medical-device companies had bribed hospital staff over many years. Eleven of them, including Hebei Xiang'en Medical Device Trading Co., Beijing Jinkaihui Medical Device Co., Hebei Mingshi Medical Device Trading Co., and Shijiazhuang Shangdaoxuan Medical Device Trading Co., were found guilty of corporate bribery by the courts. Hebei Xiang'en Medical Device Trading Co. alone paid 20.64 million yuan in bribes to sell medical consumables at artificially inflated prices to the hospital, making it one of the companies involved with the largest amounts of bribery.
By product category, the bribery centered overwhelmingly on high-value medical consumables. More than 30 types were involved, with about 174 million yuan in bribes linked to products including commonly used implantable medical devices such as intraocular lenses, drug-eluting cardiac stents, rapid-exchange balloon dilation catheters, cardiac pacemakers and bioabsorbable medical membranes. Ten types of drugs were also implicated, including naproxen sodium for injection, oxiracetam injection, lansoprazole for injection and nalmefene hydrochloride injection. Drug-related bribes totaled about 10 million yuan. Bribes involving medical consumables accounted for more than 90 percent of the total, highlighting high-value medical consumables as a major area of corruption in pharmaceutical and medical-device procurement. The notice did not disclose the names or specific positions of the hospital staff involved.
The NHSA also examined the incentives behind such bribery. Pharmaceutical and medical-device companies used bribes to influence doctors' prescribing and use of medical consumables, directly harming patients' interests. On the one hand, companies deliberately inflated their prices to create room for kickbacks, while publicly disparaging centralized procurement programs for drugs and medical consumables and promoting the misleading notion that higher-priced products are of higher quality. The aim was to get overpriced products tied to bribery schemes into hospitals. On the other hand, pockets of corruption had taken root within some medical institutions, where multiple staff members repeatedly accepted kickbacks from dozens of pharmaceutical and medical-device companies over many years. In this case, the corruption persisted for more than a decade, reflecting long-standing weaknesses in ethics training, internal controls and oversight at the hospital. These entrenched networks of illicit interests in pharmaceutical and medical-device procurement drive up healthcare costs, which are ultimately borne by public health insurance funds and patients, undermining efforts to ensure that the savings generated by centralized procurement are passed on to the public.
The NHSA said it had transferred all case leads to the Hebei Provincial Healthcare Security Administration. The information will be used for two key purposes. First, all companies implicated in the case will undergo a pharmaceutical pricing and procurement credit evaluation. Companies found to be in violation could face penalties, including restrictions on having their drugs and medical devices listed on the procurement platform and price controls. Second, the cases will serve as key leads for an unannounced inspection of the hospital designated by the healthcare security authorities, with further investigations into potential violations involving medical insurance.
